PLEASE READ THE STEPS

For course codes: FYSB08, FYSP01, FYSP10, FYSK04, FYSM34, FYSM64, and MAXM30

Step 1: Download the PDF file Download Download the PDF file and fill it electronically (no image editing, use pull-down menus to select course code and division).

• use Acrobat Reader, Okular or other PDF tool
  
  • Do not use browsers like Firefox to fill the form, they do not provide all features
• make sure to type the supervisor's full name and e-mail

Step 2: Send it together with the project plan by e-mail to the Diploma Office.

• Send the unsigned form

Step 3: Print the form and sign it, together with your supervisor.

• Hand the signed form to the diploma works coordinator, preferably during the intro meeting
• A scan of a signed form is acceptable if you can not hand it physically
